Cross Contamination Survey

The City of Athens is currently distributing Cross Contamination Surveys, which must be completed and returned no later than June 15, 2025. The Illinois Environmental Protection Agency (IEPA) requires this survey every two years as part of state regulations to help protect our public water supply.
What is Cross Contamination?
Cross-contamination (or "cross connection") refers to any actual or potential connection between a potable (drinking) water supply and a source of contamination or pollution. These situations can pose serious health risks and can occur, for example, when backflow from a boiler enters the water system. Identifying and preventing these situations is critical for keeping our water supply clean and safe.
What Do You Need to Do?
If you receive a survey, please complete it and return it to our office by June 15, 2025. You may:
- Mail it back to us at 210 Dottie Bednarko Dr., Athens, IL 62613
- Email the completed survey to cityofathens@athensil.gov
- Drop it off in person or drop box at our office located at 210 Dottie Bednarko Drive
